Steps to Receiving Services from DSS
  Complete the Admissions Application for Stark State College of
    Technology.

  Contact the Admissions office at 330-966-5450, to schedule an
    appointment to take the college entrance exam: the “COMPASS”
    test. If you need special accommodations for this test, please
    inform the admission office when you make your appointment.

  Complete the free application for federal student aid (FAFSA).

  Complete the DSS Student Application in its entirety.

  Provide Verification of Disability from an agency, physician,
    psychologist, LD Specialist or other licensed professional to the
    DSS office. Documentation must be current. Click here to
    download the Physician Verification form or obtain in Room
    S307. The Physician Verification form must be signed and dated.

  If you need assistance is requesting documentation from
    previous colleges, high schools or another agency, we can assist
    you. You can download the Request for Information form,
    complete and mail it to the appropriate address.

  Schedule an appointment with a DSS counselor in Room S307
    or call 330-494-6170, Ext. 4423 or 4754. Bring your DSS
